1. Assumption of Risk & General Release of Liability

I, the undersigned parent or legal guardian of the participant, acknowledge and agree that participation in TACK 3 Camps involves inherent and unavoidable risks, including but not limited to physical contact, running, agility drills, strength and conditioning activities, organized competition, and other football-related activities.

I understand that these activities involve risks of serious bodily injury, including but not limited to sprains, fractures, concussions, permanent disability, paralysis, and in rare cases, death, as well as risks associated with travel, weather conditions, facility conditions, and equipment use.

I knowingly, voluntarily, and freely assume all such risks, both known and unknown, even if arising from the negligence of TACK 3 Camps, its owners, directors, employees, coaches, volunteers, or affiliated facilities.

I hereby release, waive, discharge, and covenant not to sue TACK 3 Camps and all affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to participation in camp activities.

2. Concussion & Head Injury Acknowledgment

I acknowledge that participation in football-related activities carries a significant risk of head injury, including concussions and other traumatic brain injuries, which may result in long-term or permanent effects.

I understand that TACK 3 Camps implements safety procedures intended to reduce risk; however, these procedures do not eliminate the risk of head injury.

I agree that if a concussion or suspected head injury occurs, the participant will be immediately removed from participation and may not return without appropriate medical clearance.

I affirm that I will disclose any prior concussion history or relevant medical conditions prior to participation and accept responsibility for ensuring medical clearance when necessary.

3. Indemnification & Hold Harmless Agreement

I agree to indemnify, defend, and hold harmless TACK 3 Camps, its owners, employees, coaches, volunteers, and affiliated facilities from and against any and all claims, liabilities, damages, costs, or expenses (including attorney’s fees) arising from the participant’s involvement in camp activities.

4. Photo, Video & Media Release

I grant permission to TACK 3 Camps to photograph, video record, or otherwise capture the participant during camp activities.

I authorize TACK 3 Camps to use such images, video, and recordings for any lawful promotional, marketing, social media, website, or advertising purposes without compensation or prior approval.

I waive any right to inspect, approve, or claim ownership of such media materials and release TACK 3 Camps from any claims related to their use.

5. Severability & Enforcement

If any portion of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect to the maximum extent permitted by law.
